Trying to identify a plant

Does anyone know what kind of plant this is? These flowers grow out from the side of the leave, and also, some seeds fall from the leaves too, but they sprout above the ground. A friend gave it to me, but I have no idea what it is.

The bloom looks a lot like one on an Epiphylum. Also your description fits.
