dogwood tree

My beautiful dogwood tree has bloomed profusely, but now looks as if it may be in decline. The leaves are not coming out as they should. Just a few at the ends of some of the branches. We are in zone 8, and the tree is about 10 years in the same spot. Seems to have been growing well until now.
It is in an azalea bed that was top-dressed last summer. However, I don't think we changed the grade enough to damage the roots of the tree. Any suggestions?
I will try the Dirt Doctor's sick tree treatment, and hope.
