Manzanita plant spots and yellowing

Hi,
Please help me with this manzanita plant issue and what I should / can do? This is in Northern San Diego County, there are spots and leaves yellowing.

I see disturbed dirt around the base of the plant, was that intentional or do you have ground squirrels? Most problems with manzanitas are from water- usually too much water, and they should not get fertilized. It almost looks like a root rot...
