Japanese maple problems

Somerset County, NJ(Zone 6b)

I recently discovered that my japanese maple has leaves shriveled up on one branch and when I looked closer saw that there was bark damage. I had another japanese maple that appears to have suffered a similar fate last fall and is now completely dead (pictured in the last two photos). What is wrong?
