It's been tough for my little tree. The leaves have fallen off and the upper part of the trunk seems to be dead. When I scratch the lower part of the trunk it is green. I am unsure of what to do with the soil / planting material. It is a potted indoor plant. I have really failed this little guy. Please help me save him.

Citrus thrive with even moisture. The soil needs to have good drainage so you can water it often enough yet not keep it too wet. The roots need oxygen as well as water.
As an indoor plant Citrus can get a lot of different pests. Some might be severe enough to make it lose its leaves.
Citrus normally live outdoors, in full sun. Indoors they will need as much light as you can give them. I don't think I would place it in a hot south facing window, though.
At this point it has no leaves and I am worried for it. The upper part of the tree seemed dead so I cut it off. Was that to hasty?
If you scratched away a little bark and saw it was dead, then it is good to cut off the parts that are dead.
