Apple tree disease

Could anyone tell me why my apple tree is dying a slow death? The bark turns black and tends to pull away from the tree. The smaller limbs attached to the blackened limb die. I have cut away the bad limbs only to have the same thing happen again and again. HELP!!!!!!!!!!! The tree is a dwarf golden delicious that I planted about 30 years ago.
