Every once in a while, we pick a carrot that looks like this. This is the most extreme one we've seen yet - normally it's just one or two holes. It's definitely not on every carrot, just the occasional one. Can anyone tell me what pest might be responsible for this?

Mice?
We considered it being some kind of mammal, too, but the chew marks can be so far under the soil, and there don't seem to be any holes in this bed (we have squirrels who like to dig around in other beds, but we never notice holes in this one). I wondered if it could be some sort of grub. Would that make sense?
carrot weevils?
I just looked up carrot weevils and it looks like a win! I wonder if other white grubs could/would do this kind of damage. We have some larger white grubs in some of our beds - I think they're Japanese beetle larvae. So many grubs! We need a duck.
After paying an exorbitant amount of money for mealworms to feed hungry bluebirds, I did some research online and found they like carrots.
Perhaps you have something similar to mealworms/beetles eating your carrots. Tenebrio molitor, a species of darkling beetle.
