Zebra Iris Rhizome Diminishment. PLEASE HELP!!!

I have had (what I imagine to be) a Zebra Iris for 3 years and it has never bloomed. I kept it in a spot that I imagine was too shady for the first 2 years, but I didn't know what I was doing at all then..... This past Late Summer/Early Fall I finally dug it up and replanted it in a sunnier spot. When I dug it up it still had its beautiful veregated fan of leaves, but after replanting, the fan got much smaller (trauma?). This Spring it came up again, but only a couple of leaves sprouted, and have only gotten to about 5 inches high, while the other Irises in that same spot are ready to bloom. I was concerned and so checked to make sure I had not buried the rhizome too deeply, when I noticed there was hardly a rhizome at all! It always had this beautiful solid knot of a rhizome, and now it seems like there are only two tiny little knuckles and from out of them is where the couple of leaves sprouted. Where did the rhizome go???? Did something eat it? None of the other Irises had any issues...

Note: I am guessing it's a Zebra Iris after comparing its leaves to pictures I found on the internet. I know it's supposed to have a blueish bloom, with a very traditional shape to it. This is a medium-sized plant, a bit smaller than the Japanese Iris plants I have (they are almost blooming).

Thank you.....I did plant them that way. What I was wondering is why the rhizome diminished after the replant....But thank you for your response. I don't know why no one is really answering....Hm.....
