Dwarf Navel Orange Tree Disease

I'm hoping someone can help. My 3 year old tree has developed leaves and branches with tiny white/grey dots and some black stuff on the leaves. Blossoms and new growth are late and scarce this year.

See photo (sorry for poor camera). What is the unhealthy tree's diagnosis, and, how do I treat it?
