Echeveria/graptopetalum/pachyphytums....Plant ID help?

I am new to this forum and not sure if I am posting in the right place--I hope so! If not, I apologize. I joined to find help, hopefully, for my poor plant (see picture). My mother gave it to me after it had spent a summer outside potted on a patio in her garden. She doesn't remember where she got it, what it's called, and thinks, but can't be sure, that she added soil (I don't know what type) to it when she potted it....Now it is in my New York apartment, and seems to be suffering quite a bit. It is leggy, loses quite a lot of leaves generally, dropping them instantly if I even barely touch or move the plant. The leaves often turn yellow before they fall. I would like to figure out what kind of plant this is so that I can determine whether I should repot it in different soil, if so in what type of soil, what type of light it should be receiving and how much, and whether it should be fertilized, etc. It has now been in my apartment for nearly one full year, during which time I have moved it to just about every spot in my apartment. The apartment is all glass windows on two sides, the south and east facing sides, so is unbelievably sunny and hot. During the day when I'm at work, even if I close most of the shades, it has been at least 90-100 degrees in the apartment this summer. For some time I was convinced that the sun was scorching my nameless plant, so I moved it a little farther from the window so that it received a ton of bright light throughout the day, but no direct light. That seemed to make it more leggy, so I have now moved it back to the windowsill. Any thoughts on the naming or care of this plant would be GREATLY appreciated! It is so very hard to grow plants in apartments in Manhattan : ( I hope someone can help my poor plant!

I think if you re-posted over in the cacti & succulent forum you'd have better results.
