In setting planting depth, does mulch covering count?

If I plant bulbs which are supposed to be planted 3" deep...

(1) Is that from the top or the bottom of the bulb?

(2) If I add an inch of mulch (or compost), do I just plant it 2" deep in the soil?

Excluding mulch, dig a 3" deep hole.
