My Ginkgo biloba "Mariken' standards have bolted - just like lettuce in the heat of summer. Anyone else experience this? It's their third year in the ground and now I'm not sure if they have reverted or what.. All growth is above the graft line and area of long growth and larger leaves is from the mass of tighter smaller leaves, not elsewhere. I'm flummoxed

It appears the tree has no central leader by looking at the photos. The tree made a nice growth spurt but went sideways. I work at a tree farm and these periods of fast growth happens in many of our species. We have grown ginko in the past. Oaks, maples, lindens, and other trees will make these growth spurts.
Some judicious pruning, perhaps establishing a central leader if there isn't one via a stake and some green tape to secure it, should get your tree going in a good direction.
