My Clematis has looked like this for the past few years with dead leaves throughout and especially at the bottom. Can someone tell me what is happening and if there is anything I need to do to help it? Thank you.

Looks like a bad case of clematis wilt. This is a fungal disease that infects the foliage and vascular tissue of the vines. You may be able to save the plant by cutting it off at ground level (all of it, even the vines that are not dead). Then dispose of all the debris and remove any fallen dead leaves and adjacent mulch, etc. (do not compost).
