Picture taken by a friend in Thunder Bay - she says the specimen is about a half inch long, appears to be a moth and they are quite numerous. She also says that It looks like it has some kind of appendage coming from below the head. Could this be a front leg?

This is a moth in the family Alucitidae, they often are referred to as "many plume" moths. The object below its head is a front leg, its other front leg is missing. It is in the genus Alucita - see http://mothphotographersgroup.msstate.edu/species.php?hodges=2313.1
