Identified in BugGuide.net as Smicridea sp. Not a moth after all. It's a Netspinning Caddisfly
I cannot located but 4 images of Smicridea on the internet and there are dozens of species.
So unless someone has a book with the species pictures/description, I will consider this one solved.
Although the images of Smicridea dispar on bugguide appear very similar to your specimen, the verbal description of Smicridea fasciatella in the Smithsonian publication "Studies of Neotropical Caddisflies, XVII: The Genus Smicridea from North and Central America (Trichoptera: Hydropsychidae)" seems closer to the mark - see http://tinyurl.com/3r2m3sd for details. According to this publication, there are five species in this genus reported from Arizona.
I agree with you, it says "three transverse bands of white hair", and the diagram shows it on page 14, figure 19.
