These appear to be aphids; the white objects look like shed 'skins' (exuviae) of aphids. The species Hamamelistes spinosus causes the leaves of river birch to be distorted and deeply puckered and to develop a reddish coloration. These aphids develop on the undersides of the leaves in gall-like pockets,and although the leaves may be very unsightly, the aphids do not appear to cause the tree serious damage.

You probably do not, but just to be sure, check with your county agricultural extension service office.
Thanks again for pointing me in the right direction. Apparently they are spiny with-hazel gall aphids.
