I'd heard it had to "Y" first. happen ..but now I think the "Y" and the flower... can happen together..all at once.. in the same leaf division/bloom structure.. infact... here the bud comes out of the crotch formed at the time of the " Y " .. and thus is actually below the " Y "..lower than these limbs...
