Beginner Gardening: my new pitcher plant pitchers are dying, 0 by the_malteser

I bought it just before Christmas when it had lots of mature pitchers on it. As they died I removed them, since then new pitchers continue to be produced, as they open I make sure that I pour a small amount of soft water into them, as instructed. But as they get bigger the top of the pitcher turns brown and shrivels as you can see from the picture. Any ideas about what I may be doing wrong? My lovely plant is otherwise doing well, I've taken some cuttings which are doing well.
