I have had this Hoya for ??? 5-8 years. Don't think it has ever been re-potted (maybe once a few springs ago) there are a few "tendrals" that have much more curl in the leaves than the rest of the plant. Hopefully the pictures will aid in Identification. I would love to know how to re-pot it best. It is neglected because it has lived so long in this pot and flowers every summer so I have been scared to re-pot since I don't know its soil needs
???? What is this & Care needs - many years in same pot
That is a hoya called Carnosa, Someone will be along shortly I'm sure to give the exact name. Very tough plant. Likes to be root bound also.
Looks like Hoya carnosa 'Tricolor' to me, common name "Krimson Queen" : http://davesgarden.com/guides/pf/go/57703/
I have found that hoyas kinda like being root bound. If it's doing well I would just leave it alone ... maybe top dress with fresh potting mix if it looks like any of the soil has washed away and roots are exposed.
There's a lot of information in the "stickies" at the top of the hoya forum: http://davesgarden.com/community/forums/f/hoya/all/
Check out the general information thread and other links.
Edited to say: I am not a hoya expert but there are a few over on the hoya forum who would gladly answer any questions regarding your lovely plant! I may be wrong about that ID too so if you post over there you will get ideas or confirmation from other folks.
For my hoyas I use a mixture of Miracle Grow potting soil (all I can find locally) and lots of perlite and orchid bark. It is a very chunky, well draining mix because I find most hoyas don't like staying wet.

i would keep it in the pot and grow it in lower light...and grow it dry.
I agree with AlohaHoya about growing that one in lower light. Too much sun will scorch the leaves ... I know from experience on that issue! If you've had your plant for 5-8 years you must be doing something right, it looks good!
All the carnosa family really prefer and will do better in lower light and grown dry.
so it sounds like my neglect is what has kept this plant alive! It winters in the bathroom at a N window.
Here is the flower
