Gray screen with black squares?

Anyone know what would cause a dark gray screen with a black square or rectangle to appear before new pages load? The square or rectangle appears in random places. I keep trying to capture a screen shot, but it appears and disappears so fast, I haven't been able to. Sometimes, instead of a gray screen, it's solid black. And once, a white square appeared on the gray screen.
This doesn't happen all the time, either!

It just happened again, right after I hit "Preview", stayed on for a split second, then the DG page appeared.
