I've seen this plant in Costa Rica. It was in the rainforest near Monte Verde. The flower is fused to the midrib of the leaf. It was in a very cool/wet cloudforest/rainy area.

You may want to post this in the Orchids forum. It looks like a pleurothallis to me.

It is a from of pleurothallis I am no orchid expert so it may take a lot more to get the true species name.

Brian's a little up on me. I was going to suggest orchid (I know some types bloom like this), but I have no clue as to a genus.
