how to sew button on pillow?

I want to put a large covered button in the middle of two of my decorative pillows. Do I stuff them and then sew the button on thru both sides of the pillow or do I sew the button on prior to stuffing or . . .. . I want it to have a kinda gathered look around the button. and that is what I am having trouble figuring out.
