Sending HTML email?

I have written a page in publisher, that I want to send as an email through outlook. Publisher allowed me to save it as a web page, but says that "to correctly process your form, you must fill in the action box with a program from your ISP or choose a different data retrieval method." Then it asks if I want to specify the action now? I don't know, do I? And if so, what? I don't want to publish this as a web page, I am just trying to create a "fill in the blank" kind of form I can email out to clients that is interactive enough they can click on the check boxes, and send it back.

Help?
