Iroquois is a foot-licker

Iroquois has done this before, but usually he stops on his own, or with some help from medicine the vet provided called Synotic which is a fluocinolone acetonide and dimethyl sulfoxide preparation. We just put it on the "hot-spot" several times each day and he stops. This time, not working. The underside of his other foot is much worse, he looks like he has mange. I have examined the feet and find no thorns or any wounds of any sort. I wondered if anyone has similar experiences and has found a solution. Of course I've thought about stress and thought about likely stressers and can't figure it out.
