keeping records of dog shots

If I do my own dog shots, what is a good way of keeping record besides writing it down on paper, I need some kind of form, especially to give my customers who buy my pups, before I've just kept a record on paper and given them that paper, it's not really professional looking, does anybody know where I can get a form off the internet to keep track of my dog shots, wonder if I can call my vet and he'd have something I can use?
