We have had this tree for over 10 years. It has never flowered or fruited. Any help on the identification would be wonderful!

Rachel is right. There's a number of large Scheffleras but S. actinophylla is one of the most common in cultivation.
Can't locate any of my photos of the flowers but this is a budding inflorescence.
Yep, they are popular house plants, but it probably won't ever bloom indoors. Maybe a big one like that would put on a flower if you wrestle it outside for the summer. They take full sun here. Yours looks like it needs more light to me. A bit leggy.
Down here they are invasives that seed themselves, grow to 20ft., have 2ft. long bright red inflorescences and can be a right nuisance.

When was the last time you did a soil change? If it has been over a year, I would do it.
Wash and rinse your leaves with warm soap and water. 3-4 drops of Dawn to a gallon would be great. The stomata of the leaves are being suffocated by the dust preventing them from proper transpiration to the plant, resulting in lackluster growth.
It appears your watering schedule is great.
