This Crinum takes my breath away. When it first started to emerge I noticed this blotch or imperfection. I was bummed thinking that when it opened it would have a hole in it. Yesterday I was thrilled when I noticed that the blotch looks like a little heart. I've taken quite a few photos.

Dawn, thanks for your reply---It wasn't a cutout---it was an impression---I think it was caused by pressure of the other flowers pressing against it in the head of the flower stalk.
