I can't find another one and I really want this one. Where would I get one? Does anyone else have this particular one? Do you think it can be saved? What kind of fertilizer to use?

What's wrong with it?
When we moved in this house, it was completely overgrown and covered up. I couldn't even tell it was there. It has been deprived of water and sunlight. There is only half that is still alive and has a few green leaves on it. I don't think it can be saved but I would try if someone would tell me what to do.
It looks like it has new growth on it! I would trim off everything above the new growth so that the tree doesn't waste energy on the dead stuff. Continue to water it regularly. I wouldn't water daily, but maybe 3 times a week, and water deeply.
Do you have any good compost you could spread out around the base. That would be good for it.
They're tough...
I would arrange the mulch just like for a tree - leave a hands width bare around the trunk, and then make sure the rest is about a hands width deep. And yeah, water and compost. If you've got a soaker hose or can otherwise arrange for slow watering, that probably would be most effective. Water around the drip line, not right at the trunk.
They can be propagated from cuttings. If you're real attached to this particular plant, you might take a few and go ahead and get a possible replacement started.

I cant add any advice but I m shocked to see a CM like that.. Ive seen some that were caught on fire by a house fire come back bigger and better with no special treatment and the ones Ive cut down 3 yrs ago are still sending up suckers I cant get rid of..I didn't think you could kill them even if you tried..at least I cant...

Oh!! Something else! It could be planted too deeply. Pull back the mulch down to the root flare (base of tree) so it can breathe! Then, you might want to try the sick tree treatment mentioned here:
