It almost sounds like your roots are rotting. I'd pull it out of the pot and examine the roots at this stage. If they are rotting/rotted you can cut the stem off at the point of rot and start it once again. You may have to callus the cut before trying to repot it though.

I've had mine for many years without any serious trouble in the same pot. I water it sparingly in winter but in summer I water it often outdoors.
