Beginner Gardening: following flower id, now i would love a disease ID, 0 by vipalpinetours

Help! i added the photos of my favorite unknown plant, which is the white bell shaped lantern with the red flowers at the end. but it has a disease. ive had it 5 years, but last fall, thought it was dead. i trimmed it back it bloomed vigorously and as it grows,the leaves curl, and they lose their colour pigment, fading to cream/yellow.
